85yo frail 65kg patient for hand mass excision, recieved US-guided infraclavicular block awake, 28ml of 2:1 mixture Mepivacaine 2%: Ropivacaine 0.5% with negative aspiration q5 ml injected, altered mental status with fluctuating responsiveness and facial motor symptoms developed approximately 15 minutes post-injection, no cardiac symptoms or EKG changes, 100ml Intralipid bolus given with complete resolution of symptoms within 15 minutes, continued low rate infusion and observation overnight
