19 yo, 54kg, healthy female with history of lumbar microdiscectomy 1 year prior had recurrence of low back pain. Pain was axial and non radicular. On exam she had tight bands of muscle in the lumbar paraspinals. She underwent ultrasound guided lumbar trigger point injections targeting 4 trigger points. A total of 10ml of 2% Lidocaine mixed with 5ml of 1% Lidocaine (total 250mg) was injected in divided doses. The patient did well immediately after the procedure but about 10 after she began feeling light headed. She developed tachycardia and became hypertensive and uneasy. She developed perioral numbness and numbness in her throat and became somnolent but never lost consciousness . 100% Oxygen was administered. She improved spontaneously after about 30 min of monitoring but went to the ER due to persistent tachycardia and throat numbness. Lipid emulsion was administered in the ER and she recovered fully.
